5 Simple Steps
Complete our Online Inquiry Form.

Complete the Online Application Form. Current families registering siblings can log in to FACTS, choose Apply/Enroll, and select Application.

Submit the following academic records and documents:
Recent standardized test results (grades 1-8)
Most recent report card (grades 1-8)
Birth certificate
Tennessee proof of immunization form
Current Teacher Recommendation form (grades K-8)
Contributing Catholic Form (if applicable)

Schedule a interview and admission testing (grades 1-8).

The Admission Committee will review your application. Upon acceptance, complete and return the Registration Forms, pay the $150 registration fee, and sign the Continuous Enrollment Contract.
